Lost A Certificate?

QNC is required to retain class records for six years.  We've kept everything since 1996 - but sadly, the electronic data base we developed was lost in a computer meltdown, and all we now have is the original paper files. To request a replacement certificate, CONTACT US with the month and the year you took the class.  Include address and phone updates in case you've moved.  We'll search the files, verify your credentials, and issue a new certificate for the nominal fee of just $15.

Original Certificates are the property of the caregiver completing the class.  Caregivers should build their credentials file by protecting and organizing their original training certificates.  I recommend a binder with acid-free plastic document protectors. 

Copies (not originals) of training certificates should be provided to employers. 


Quality Nurse Consultants

Jo Lyn Cornelsen RN established Quality Nurse Consultants in 1995, and began training AFH caregivers in 1996 in Clark County, Washington.  

QNC became Clark County's first approved Independent Trainer for the classes "Fundamentals of Caregiving", "Modified Fundamentals of Caregiving", and "Nurse Delegation Training for Nursing Assistants".  Many local nurses who have become trainers attended Jo Lyn's early classes. 

Jo Lyn also taught "Ensuring Quality Care", Oregon's Adult Care Home Training, in Hillsboro, OR from 1995 through 2004.

At last count, over 3000 students have attended Jo Lyn's classes since 1996.  We've stopped counting!
